Clinical Documentation and Evaluations

Clinical Documentation and Evaluations

Clinical documentation and evaluations may be completed when clinically appropriate and supported by assessment.
This may include forms or letters related to:
  • Disability
  • Emotional Support Animal Evaluations
  • FMLA
  • Medication or diagnosis documentation related forms
  • Pre-procedural or pre-surgical psychological evaluations
  • Return-to-work needs
  • School or work accommodations
  • Treatment verification
  • Worker’s compensation forms

Completion of documentation is not guaranteed and depends on clinical appropriateness, evaluation findings, available records, and the nature of the request. Some forms may require scheduling a separate visit, review of prior records, or coordination with other providers.
