Self-Pay Appointments
Schedule directly without using insurance. Self-pay appointments are available for patients located in Arizona, Florida, Ohio, and Washington.
Superbills are available upon request for possible out-of-network reimbursement. Reimbursement is determined by your insurance plan and is not guaranteed.

Insurance-based appointments are available to eligible Florida patients enrolled in participating plans.
Coverage, eligibility, copays, deductibles, and other patient responsibilities vary by plan and are confirmed during scheduling.
