Frequently Asked Questions
1. What services does ClearLane Behavioral Health provide?
ClearLane Behavioral Health provides psychiatric evaluations and medication management, psychotherapy, substance use treatment, ADHD care, and treatment for concerns such as anxiety, depression, insomnia, and other behavioral health needs. Limited family medicine services may also be provided when associated with psychiatric concerns and appropriate for telehealth.
2. Are appointments offered through telehealth?
Yes. Appointments are completed through secure video visits, allowing you to receive care from the comfort and privacy of your home or another appropriate private location. You must be physically located in a state where your ClearLane clinician is authorized to provide care at the time of your appointment.
3. What states do you currently serve?
ClearLane currently serves patients in Florida, Arizona, Washington, and Ohio. Available services, insurance participation, and prescribing options may vary by state and are subject to applicable state and federal requirements.
4. Do you accept insurance?
Yes. ClearLane participates with select insurance plans depending on your state and individual coverage. TRICARE is accepted in Florida and Ohio. Visit our scheduling page and select your state and insurance or self-pay to open the correct appointment calendar. If ClearLane is not in network with your insurance plan, contact the practice, as additional options may still be available.
5. Will my insurance cover my appointment?
Coverage varies by insurance plan. Eligibility and benefits may be verified when insurance is being used; however, verification of benefits is not a guarantee of coverage or payment by your insurance company. Your individual copay, coinsurance, deductible, or other financial responsibility may apply. Final coverage and payment decisions are made by your insurance plan.
6. What are your self-pay rates?
Self-pay initial appointments are $250, and follow-up appointments are $150. Applicable fees are reviewed during the scheduling and registration process.
7. How do I schedule an appointment?
Appointments can be scheduled online through the appropriate booking option for your state and insurance coverage. If you are unsure which option to choose, contact ClearLane and we can help point you in the right direction.
8. How long are appointments?
Initial appointments may last up to 60 minutes depending on your needs. Follow-up appointments are typically 15–30 minutes, depending on the type of visit and your treatment needs.
9. What should I complete before my first appointment?
Please complete all requested intake forms and provide your current medications, relevant medical and psychiatric history, demographic information, pharmacy information, and insurance information, when applicable, before your visit.
Additional records, identification, or documentation may be requested when necessary for safe and appropriate treatment.
10. Do you prescribe controlled medications?
Controlled medications may be considered when clinically appropriate and permitted by federal and state law.
A controlled medication or any specific prescription is never guaranteed before or after an evaluation. Depending on the medication and individual circumstances, additional requirements may include identification verification, review of prior treatment records, prescription-monitoring database review, laboratory or toxicology testing, vital-sign monitoring, medication counts, more frequent follow-up, or an in-person evaluation when required.
Prescribing decisions are made by the treating clinician based on clinical judgment, safety, applicable law, and the individual patient’s treatment needs.
11. Can medication be prescribed during my first appointment?
Medication may be prescribed during an initial appointment when clinically appropriate and legally permitted.
Treatment recommendations are based on your evaluation, medical and psychiatric history, current medications, previous treatment, safety considerations, and individual needs. A particular medication, controlled medication, dosage, or prescription is never guaranteed.
12. Can I request a medication that has worked for me before?
Yes. You are encouraged to discuss medications and treatments that have helped you in the past.
Previous treatment history is an important part of your evaluation; however, previous receipt of a medication from another healthcare professional does not guarantee that the same medication or dosage will be prescribed or continued by ClearLane.
Treatment decisions are made collaboratively based on your current symptoms, history, treatment goals, safety considerations, and the clinician’s professional judgment.
13. Do you provide medication-assisted treatment (MAT)?
Yes. Medication treatment for substance use disorders, including medication for opioid use disorder when appropriate, may be provided as part of an individualized treatment plan.
Treatment recommendations depend on your clinical evaluation, treatment needs, applicable monitoring requirements, and federal and state law.
14. Do you see children or adolescents?
ClearLane provides outpatient psychiatric care for teens ages 13 and older, as well as adults. Services may include psychiatric evaluations, medication management, and psychotherapy based on each patient’s needs and clinical appropriateness.
For patients under 18, parent or legal guardian involvement and consent are generally required, subject to applicable state law.
15. Can I refer a friend or family member?
Yes. We welcome referrals from friends, family members, healthcare providers, therapists, and other community professionals.
Anyone interested in services is welcome to contact ClearLane directly or schedule an appointment online.
Please avoid sending another person’s confidential medical information without their permission unless you are legally authorized to do so.
16. What is your cancellation policy?
ClearLane requires at least 24 hours’ notice when canceling or rescheduling an appointment.
Late cancellations and missed appointments may be subject to a fee in accordance with the financial and cancellation policies provided during scheduling or registration.
17. Is ClearLane appropriate for emergencies?
No. ClearLane Behavioral Health is an outpatient practice and does not provide emergency or crisis services.
The website, email, text messaging, voicemail, scheduling systems, and routine patient communications should not be used for emergencies and may not be monitored continuously.
If you are experiencing a medical emergency, call 911 or go to your nearest emergency department.
If you are experiencing a mental health or suicide-related crisis, call or text 988 to reach the Suicide & Crisis Lifeline.
18. Do I have a right to a Good Faith Estimate?
Yes. Under federal law, if you do not have health insurance or you choose not to use your health insurance to pay for your care, you generally have the right to receive a written Good Faith Estimate explaining the expected cost of your healthcare services.
ClearLane Behavioral Health maintains transparent self-pay pricing. You may request a Good Faith Estimate before scheduling an appointment or at any time before receiving care.
If you schedule an appointment at least 3 business days in advance, a Good Faith Estimate will be provided within the timeframe required by federal law. Generally, if you schedule care 3–9 business days in advance, the estimate must be provided within 1 business day after scheduling. If you schedule care 10 or more business days in advance, the estimate must generally be provided within 3 business days after scheduling. If you request an estimate before scheduling, it generally must be provided within 3 business days after the request.
A Good Faith Estimate reflects the services and charges reasonably expected at the time the estimate is prepared. It is not a guarantee that your treatment needs will never change.
If you receive a bill from a provider that is at least $400 more than the amount listed on that provider’s Good Faith Estimate, you may be eligible to use the federal patient-provider dispute resolution process.
For more information about Good Faith Estimates and your rights under the No Surprises Act, visit CMS.gov.
