Clinical documentation and evaluations may be completed when clinically appropriate and supported by assessment.
This may include forms or letters related to:

- Emotional Support Animal Evaluations
- FMLA
- Medication or diagnosis documentation related forms
- Pre-procedural or pre-surgical psychological evaluations
- Return-to-work needs
- School or work accommodations
- Treatment verification
- Worker’s compensation forms
Completion of documentation is not guaranteed and depends on clinical appropriateness, evaluation findings, available records, and the nature of the request. Some forms may require scheduling a separate visit, review of prior records, or coordination with other providers.
