Clinical Documentation and Evaluations

Clinical documentation services are available for individuals who require behavioral health evaluation, treatment verification, or completion of appropriate medical and psychiatric forms.

Documentation is based on clinical findings, available records, functional limitations, treatment history, and the specific requirements of the requesting organization.

Documentation may include:

FMLA Documentation

Clinically supported documentation for eligible medical or psychiatric leave.

Workplace Accommodations

Evaluation of clinically appropriate workplace accommodation requests.

Academic Accommodations

Documentation related to qualifying psychiatric or functional limitations.

Disability Forms

Short-term disability documentation when supported by the clinical findings.

Documentation Requirements

Completion of a form, letter, or evaluation is not guaranteed. Sufficient clinical information, supporting records, and an appropriate treatment relationship may be required.
